Navigating the Health Field After a Hospital Visit

According to the Canadian Institute of Health Information (2024), there were 2.96 million acute inpatient hospitalizations in Canada from 2022-2023. Once you are admitted to the hospital what are the next steps? In this article, we will walk you through what a typical process of being admitted to the hospital and going back home would look like. We will also discuss when you may need additional care and how Kinesiologists (R.Kin) can help in your recovery.

Let’s start with being admitted to the hospital. Once you are taken care of by a health care team, they will discuss a plan of care and help prepare you to leave the hospital (discharge). Discharge is when the health care team working with you decides that you no longer need medical care at the hospital. When you find out your discharge date, staff at the hospital will help you and your family plan ahead and support you in arranging any follow up care if needed. Examples of services you may need include home support, rehabilitation services, and/or nursing. The health care team working with you will already have done an assessment as to whether you need these external services and may connect you with their home and community care services to discuss your needs further.

Once you arrive home, you may already have a follow-up appointment booked with the health care team at the hospital, your family doctor, or other health care provider. Keep in mind that depending on your reason for being admitted to the hospital, your recovery will affect your daily routine. For example, recovering from a surgery, you may have limited mobility that can make it challenging to carry out your day-to-day activities. In this case, you may need additional care to help you regain your strength and maintain independence. Speaking with your family doctor or other health care provider is an important step to see how you are recovering and is a chance to express if you need extra support from other health care professionals.
